This bundle is designed for Occupational Therapists working in a paediatric setting - in particular, clinicians who are seeking clinical guidance and support in the areas of assessment, reporting and treatment planning for clients in the area of regulation. This bundle is a guide that can compliment existing clinical supervision, and/or provide structure when working in a setting that does not have a current structured clinical process.
The Assessment Protocol and Clinical Protocols are downloadable PDFs. Save them, and refer to them when making decisions about assessment recommendations, or intervention options.
The report writing template is downloadable in .docx format (word/google doc). It includes both a master report template (overall structure) and individual assessment report 'blocks'. Use the master template on your own letterhead, and modify the content as needed. Cut/Paste the report to match the sections you use in your assessment process, or copy and paste the bits that suit you - into your own report templates.
A note that the .docx files were designed in google drive and are likely to have some formatting 'shift' - as they move across programs which may potentially decrease readibility. It is recommended you use the text and formatting appropriate with your organisation &/or remove the Clinician Collective headers/footers.
